DUTCH Plus is a comprehensive analysis of dry urine and saliva, which allows you to get maximum information about the production and metabolism of sex hormones and adrenal hormones.

By simply collecting samples at home, you can get an idea of the total daily structure of free cortisol and cortisone, as well as the overall distribution of cortisol metabolites. This test allows you to obtain additional information, including an analysis of organic acids, which allows you to assess possible nutritional deficiencies, oxidative stress, intestinal dysbiosis, melatonin levels and neuroinflammation.

With DUTCH Plus, you can also get additional information about the function of the adrenal glands, including the cortisol awakening response (CAR – cortisol awakening response), which shows a natural increase and decrease in cortisol levels during the first hour after waking up.

When is it recommended to use DUTCH Plus?

This test is recommended if you need information about:

  • the development of sex hormones and the peculiarities of metabolism
  • production of adrenal hormones (DHEA and cortisol) and metabolic features
  • daily levels of free cortisol and cortisone
  • melatonin production during sleep
  • availability of trace elements and oxidative stress.

Both panels provide the above information, but what is the difference? DUTCH Plus should be used if you want to get a more complete picture of the functioning of the hypatoloma-pituitary-adrenal axis (HPA). The cortisol response to arousal (CAR) provides an additional assessment of the function of the HGH axis and may independently indicate the dysfunction of the HGH axis. There are cases when the daily cortisol curve is within the normal range, but the CAR is not. DUTCH Plus allows you to get both views on the function of the adrenal glands.

For which patients is it best to use DUTCH Plus?

Examples of common conditions associated with less pronounced cortisol level disorders are fatigue, depression, insomnia, fibromyalgia, anxiety, inflammation, etc.

The DUTCH Plus test should be considered if patients have the following symptoms and conditions associated with adrenal dysregulation:

  • premenstrual syndrome,
  • symptoms of menopause
  • infertility
  • unexplained increase in body weight in the abdomen (apple-shaped body)
  • IBS - irritable bowel
  • muscle pain or rapid destruction of muscles/joints
  • headaches, including migraines
  • heart disease
  • high blood pressure
  • a violation of the regulation of blood sugar levels
  • anxiety
  • depression
  • fatigue
  • insomnia
  • injuries that heal slowly
  • symptoms or diagnosis of hypothyroidism.

In addition, a low or delayed cortisol response to awakening may be the result of insufficient activity of the HPA axis. These include:

  • seasonal affective disorder
  • excessive psychological burnout
  • post-traumatic stress disorder
  • chronic fatigue
  • sleep apnea
  • poor sleep in general
  • chronic pain.

Reduced CAR levels are also associated with systemic hypertension, postpartum depression, functional gastrointestinal diseases and autoimmune diseases.

An increased cortisol response to awakening may be the result of an overreaction of the HPA axis. These include:

  • glycemic dysregulation
  • constant stress associated with work (anticipation of the upcoming day)
  • pain
  • general depression

What samples are needed for the DUTCH Plus test?

The DUTCH Plus test requires four dry urine samples and five saliva samples. These samples should be collected during the day from waking up to going to bed.
